Accessing healthcare can be challenging, especially in a system where demand continues to grow and patients are navigating multiple steps to get the support they need.
While there is no single solution to every challenge in primary care, we regularly review our processes, tools, and workflows, guided by feedback from patients and our care teams to strengthen communication, improve access, and enhance the overall care experience.
We’ve worked hard to create modern clinic spaces, staffed by caring physicians and supported by technology that helps patients navigate care more easily.
We also want our patients to connect with their healthcare team in ways that fit their lives, whether that's through online booking (available 24/7), secure messaging, virtual care when appropriate, or in-person appointments across our three Toronto clinic locations.
Behind the scenes, we're actively updating the digital tools that support these experiences.
Our ongoing electronic medical record (EMR) improvements make it easier for patients to request prescriptions and medical forms, access vaccination records, receive preventative care reminders, and navigate healthcare services with fewer barriers.
These improvements are especially valuable for patients balancing work schedules, caregiving responsibilities, mobility challenges, or other circumstances that can make accessing care more difficult.
By reducing unnecessary barriers, patients can spend more time focusing on their health and less time figuring out the healthcare system.
We're also proud to participate in Ontario's Primary Care Action Plan, which helps us expand access to family physicians for patients who have historically faced greater challenges connecting with primary care.
As our team grows, we're excited to have family physicians of all genders accepting new patients, and we're actively recruiting additional doctors to meet the growing needs of our communities.

Every healthcare decision depends on having the right medical information at the right time.
But in today's healthcare system, that information is often spread across different clinics, hospitals, pharmacies, and specialist offices.
When patient information isn’t easily shared between care settings, healthcare providers may not always have the full picture of a patient's health. In some cases, important details and results may not be available when they’re needed most, making it harder to provide seamless care over time.
For patients, this can sometimes mean repeating their health history, sharing the same information with multiple providers, or helping connect the dots between members of their care team.
That's why one of our biggest priorities is making healthcare information easier to access, organize, and use throughout primary care.
Through our ongoing <Better Care/> campaign, we're improving our shared open-source EMR system and exploring new ways to help our care teams bring existing health information together more efficiently.
One example of this work is our ongoing effort to reduce missing medical information — helping ensure important details don't get lost as patients move between different parts of the healthcare system. You can learn more about our work to close the loop on results here.
We're also collaborating with community partners to identify opportunities for better integration across healthcare information systems and stronger coordination between care providers.
Beyond making healthcare information easier to organize and share, we're also committed to making it easier for patients to understand.

Great primary care means helping people stay healthy — not just treating illness when it occurs.
Our physicians place a strong emphasis on preventive care by keeping patients up to date on recommended screenings, immunizations, and chronic disease management, while working to reduce long-term health risks whenever possible.
We also believe that access to trusted health information is an important part of care, both inside and outside of the clinic.
That's why we create evidence-based patient resources that help people better understand and manage their health between visits.
Our guides cover topics such as vaccinations, home blood pressure monitoring, smoking cessation, and knowing when to seek urgent care.
We also connect patients with trusted online information on mental health, pregnancy and fertility, nutrition, and self-referral services available throughout Ontario.
By combining preventive care with accessible health information, we hope to support long-term health outcomes and give patients the knowledge and confidence to take an active role in their well-being.
