How to Transfer:

You are welcome to register to become a patient of Dr. Pizzolato at Magenta Health by pre-registering via the link below, then having a short intake appointment with her.

When pre-registering, be sure to select “I prefer Dr. Pizzolato at the Midtown (Avenue & Eglinton) location because I am her existing patient at another clinic” to ensure you are assigned to her.

Once you register, you’ll likely be invited by email to book an intake appointment with her.

A few important things to know:

• Magenta Health communicates primarily electronically (e.g. appointments are booked online). Please learn more about our communication policy before proceeding.

• At the moment, there is no firm deadline in place to register by.

• There are no Magenta Health staff fluent in Portuguese, only English. If you require a translator, you will need to provide your own (e.g. a family member or friend).
  

What happens if I don’t register and have an intake?

• You must have a short intake with Dr. Pizzolato before she is able to consider you a patient at Magenta Health. It may take some time before you’re able to schedule this intake appointment, so please try to schedule as soon as possible.

• After February 22, 2023, if you haven’t had this short intake, you will not be a patient of Dr. Pizzolato and will not be able to obtain care from her or her new colleagues. We are not allowed to make exceptions, our apologies. If you require care, we recommend a walk-in clinic. As mentioned, there is currently no firm deadline to re-register with her at Magenta Health via the link above, you can continue to do so after February 22, 2023.
