How to Transfer:

You are welcome to register to become a patient of Dr. Morin at Magenta Health by pre-registering with us, then having a short in-person intake appointment with her.

You can pre-register here. To ensure you are assigned to Dr. Morin correctly, please ensure you answer these two questions in the affirmative when the opportunity arises:

• Are you a patient of Dr. Carly Morin at Toronto West Medical? (answer ‘Yes, I’m a patient of Dr. Carly Morin’)

• Preferred Clinic Location? (answer ‘I am a former patient of Dr. Carly Morin at Toronto West Medical and prefer to move with her to the Riverside (Queen & Broadview location’)

A few important things to know:

• Magenta Health communicates primarily electronically (e.g. appointments are booked online). Please learn more about our communication policy before proceeding.

• Individuals must be comfortable traveling primarily to Magenta Health - Riverside for in-person appointments, as well as other Magenta Health locations near Midtown and the Beaches for urgent matters.
  

What happens if I don’t register and have an intake?

• You must have a short intake with Dr. Morin before she is able to consider you a patient at Magenta Health. It may take some time before you’re able to schedule this intake appointment, so please try to schedule as soon as possible. Intake appointments will start after January 1, 2025.

• After December 31, 2024, you will not be considered a patient of Dr. Morin and will not be able to obtain care from her or her new colleagues until the short intake has been completed. We are not allowed to make exceptions, our apologies. If you require care, you can see Dr. Morin’s replacement at Toronto West Medical until the transfer and intake are completed.
