REGISTERING YOUR NEWBORN WHO IS not SEEING A MIDWIFE Post Hospital Discharge

If you are an adult wishing to register, you can do so on our new patient registration page, but we recommend that you look for another clinic to see your newborn at this time as our physicians can only accept newborns of existing patients. Keep in mind that even if you pre-register yourself, we will not be able to register your newborn until after you have already completed an intake appointment.

I’M AN ACTIVE MAGENTA HEALTH PATIENT AND MY NEWBORN IS not SEEING A MIDWIFE post hospital discharge


Before you and your newborn baby are discharged from the hospital, you should receive instructions on when you need to bring your newborn in for his/her first check-up with the doctor.  This is usually within 48-72 hours of hospital discharge.  

If you wish to register your newborn baby with your Magenta Health family doctor, please complete the form below. These submissions are reviewed during business days (Monday through Friday) and are triaged by our staff with high priority. Once received, we'll contact you by email and phone to let you know of a date & time for your newborn's first appointment. You'll need to register your newborn for OHIP first though.


Lastly, the above information only applies to newborn babies who have just been discharged from the hospital

  • If you have an older infant or child that you wish to register as a patient, please go through the standard patient registration process.

  • If you are a current patient who has already filled in the form below, and has received confirmation that your child has been registered, please use the ‘Well Baby’ option on your physician’s booking page in order to book their first visit after Midwife Discharge.


* Although the purpose of this process is for your newborn to become a patient of one of the family physicians at Magenta Health, please note that completing the above form alone does not make your newborn a patient yet.
