Notes from your physician

In order to ensure patients who require care have timely access, your physician has requested that patients do not schedule appointments for the sole purpose of requesting sick notes (i.e. if you do not have any medical concerns that you wish to discuss with your physician). Instead, in such circumstances, please contact your physician in writing to request a sick note. When doing so, please specify the dates that you were off and when you returned back to work. Unfortunately your physician is unable to backdate these notes.
