what’s the funding all about, anyway?
Beginning May 2025, the Ontario government has a goal of adding 305 new primary care teams across the province, connecting 2 million + people to a publicly funded primary care within four years. To achieve this goal, the province is calling for healthcare organizations to submit proposals for funding to assist with expanding the care they deliver (i.e. Interprofessional Primary Care Teams, or IPCTs). This first round of funding was announced in June 2025 - if your application has been approved, or if you’re considering submitting a proposal for the next round, we’re ready to support your IPCT vision and become your partner.
The provincial objectives for the IPCT funding are:
Attach More Unattached Patients
Help connect more Ontarians — especially in high-need areas — to a regular primary care provider and team.Expand Interprofessional Team-Based Care
Strengthen or create primary care teams (family doctors, NPs, nurses, social workers, etc.) to provide comprehensive, collaborative care.Improve Patient Access & Avoid ED Usage
Reduce avoidable hospital visits by targeting unattached patients, reducing barriers to care, and ensuring patients receive timely, high-quality, coordinated services.
About your team
You’re looking to grow your organization by either building a new primary care team, or expanding your current one. You’ve experienced frustrations with operationalizing this, and frankly, you feel a bit overwhelmed about where to begin. You know attaching more patients is important, but even with the funding, it feels daunting.
In order to be eligible for the funding, your organization should be able to service an eligible postal code community, looking to create or expand one of the following team-based models*:
Family Health Teams
Community Health Centres
Nurse Practitioner-Led Clinics
Indigenous Primary Health Care Organizations
*Note: your organization itself does not necessarily need to be a FHT, CHC, NPLC or Indigenous organization.

There have been many months in our history where we have onboarded more than 500 new patients. Our infrastructure and experience allow us to scale responsibly in alignment with the provincial goals.

Accessible, Patient-Centred Care - Enabled by Smart Digital Design
At Magenta Health, we ensure timely and equitable access through multiple patient-centred features: online booking, after-hours care, and same- or next-day urgent appointments, with seamless integration of virtual options available when appropriate.
We actively educate patients on how to access the right care at the right time, helping reduce unnecessary emergency department visits. In addition, our Ontario Health-supported investment in a custom-built patient portal (our “Digital Front Door”) has transformed the way patients engage with our clinic, enabling intuitive navigation for queries, triage, and booking.
Our clinics have experienced the pressures of increasing rent and staffing costs, ever increasing patient expectations, the weight of pressure from every stakeholder, and the challenge of recruiting & retaining strong family doctors. We have experience working with hospitals and community care agencies on health system design, governance, and collaborative care projects, as well as working with Ontario Health funded projects.
