Book a New Patient Intake Appointment with Dr. Anna Piekos

This page is for invited patients to schedule a new patient intake appointment with the above physician. A link to this page is normally sent to you by email after you complete our standard registration process. If that's the case for you, please read through all of this information (we know there's a lot, but it's important!), and then use the form at the very bottom to schedule your first appointment.

If you have not been invited to schedule an appointment, but would like to enroll as a patient, please complete our registration form on our registration page, and we will be in touch as soon as possible by email.

Purpose of Appointment

This appointment is not meant for providing medical care, but to let you have a chance to meet Dr. Piekos, to learn about our clinic, to complete all necessary forms and sign-ups, and for a comprehensive medical history to be taken. 

Kindly note that you will not be Dr. Piekos' patient until the patient intake appointment has been completed (e.g. you will not be able to schedule a regular medical appointment). One of the purposes of the initial intake appointment is for you and Dr. Piekos to meet, and then to take it from there. Additionally, to be clear, you will be enrolling as a patient of Dr. Anna Piekos and not of Magenta Health Inc. more generally.

Appointment Details

You will be sent by email a link to an intake form.  If you're able to complete this form in advance, please arrive 5 minutes early to complete a couple additional forms. Otherwise, please plan on arriving 15 minutes early to complete the form in person. Unfortunately, to help our doctors stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule.

Our physicians work very hard to keep to their schedule and do not double book appointments. Nevertheless, there may be times when they are running behind, for example, if a previous appointment runs long due to an unexpectedly complex issue. Kindly keep this in mind when scheduling appointments and give yourself some buffer time.

This schedule is up-to-date and complete for the next 60 days. If a convenient time is not currently available, please check back later - someone may have cancelled in the interim and additional appointment timeslots will be released on an ongoing basis.

Use of Technology

Lastly, we'll reiterate that one of the unique aspects of our clinic is our emphasis on leveraging technology to improve patient care, make us more accessible to patients, reduce the overall burden on the Canadian health care system, and enable our physicians to safely and competently provide care to more patients in our community.  

Two of the ways that this will affect your patient experience is that (i) you will be able to communicate with the clinic and your physician through email, with a view to eventually use a secure online messaging portal once available, and (ii) you will be able to book appointments online anytime, even when the clinic is closed. As compared to a traditional clinic that primarily communicates by phone, we feel that these communication tools will enable us to reduce errors, protect patient privacy, automate our processes, and improve patient care - so much so that we will be re-directing patients who phone into the clinic to submit their message or book their appointment online instead.  

In other words, by and large, you will be asked to communicate with the clinic almost exclusively online or in person.
